What you'll do

• Conduct patient assessments and develop individualized treatment and aftercare plans
• facilitate individual, group, and family therapy
• provide patient and family education on addiction, recovery, and relapse prevention
• perform crisis intervention and de-escalation
• present cases to the multidisciplinary team
• maintain documentation in the EMR
• provide case management and connect patients with community resources.
