The position provides facility-based and/or community psychiatric rehabilitation services for WES Health System. The worker supports participants in developing and retaining skills for daily living, learning, working, and socializing. Through individual support, group facilitation, community activities, and coordination with service providers, the role contributes to participants’ rehabilitation goals and access to chosen supports.

What you'll do

• Provide psychiatric rehabilitation services
• facilitate groups and individual supports
• assist with individualized rehabilitation plans and daily documentation
• plan community-integrative activities
• maintain an assigned caseload
• participate in program planning and service-gap analysis
• assist with crisis situations and resource connections
• coordinate with other service providers
• advocate for participants
• and attend relevant meetings and training.
