What you'll do

• Coordinate, monitor, and evaluate clinical and wellness services
• oversee resident assessments, care planning, and medication management
• hire, train, and mentor nurses and caregivers
• maintain regulatory survey readiness and clinical documentation
• follow up on incidents and care issues
• partner with pharmacy consultants, physicians, and external agencies
• collaborate with community leaders
• participate in care conferences, family meetings, and community activities
• advocate for residents and families.
