What you'll do

• Conduct daily clinical rounds at assigned skilled nursing facilities
• evaluate and examine newly admitted patients and establish care plans
• directly manage higher-risk, medically complex patients requiring intensive monitoring
• provide clinical supervision and mentorship to advanced practice providers
• review and sign APP charts.
