The Home Health Registered Nurse (RN) role is onsite in patients’ homes in Antioch, CA. Compensation is $70–$120 per hour. The Adult Home Health Division of Delta Bay Healthcare provides personalized care intended to support patient well-being and independent living. The nurse assesses patients, develops and updates individualized care plans, and delivers prescribed treatments to support health outcomes outside traditional clinical settings. The role also coordinates with patients, families, physicians, and other healthcare professionals, while educating patients and caregivers to help manage health conditions and reduce hospital readmissions.

What you'll do

• Assess patients in the home environment
• develop, implement, and update individualized nursing care plans based on assessments and physician orders
• administer prescribed medications, treatments, and therapies
• educate patients and families on disease management, medication administration, wound care, and preventive health
• coordinate with interdisciplinary healthcare teams
• document care and clinical observations
• monitor progress and adjust care plans
• provide emotional support to patients and families.
