The role leads the facility’s Resident Assessment Instrument process and supports accurate Patient-Driven Payment Model reimbursement. Through timely assessments, interdisciplinary care planning, quality monitoring, and compliance work, the coordinator helps align resident care documentation with clinical and regulatory requirements.
What you'll do
- Complete MDS assessments accurately within CMS Assessment Reference Date windows
- submit assessments to iQIES and resolve rejected or late submissions
- manage PDPM case mix, including clinical categories, NTA scoring, and therapy minutes
- lead or co-lead interdisciplinary care conferences
- develop and update individualized, person-centered care plans
- monitor Quality Measures and contribute to QAPI initiatives
- conduct internal MDS audits
- educate nursing staff on documentation supporting MDS coding
- collaborate with the Business Office on Triple Check and billing accuracy
- assist with survey preparation and respond to MDS-related findings.

Why this role
Monday–Friday role; current RN license strongly preferred, with an LPN who has significant MDS experience considered; RAC-CT certification preferred or willingness to obtain.
