Registered Nurse (RN) Case Manager

The Registered Nurse (RN) Case Manager role is onsite in Fort Walton Beach, FL, with compensation of $34–$42 per hour. The position supports patient care at a 267-bed hospital with stroke, cancer, cardiac surgery, and emergency services. The role coordinates patient services and resources, develops individualized care plans, and guides patients and families through rehabilitation and discharge planning. Through assessments, education, documentation, and collaboration with an interdisciplinary team, the case manager helps support patient-centered transitions across the care continuum.

What you'll do

• Coordinate services and connect patients and families with resources
• develop individualized care plans
• orient patients and families to rehabilitation, insurance benefits, discharge criteria, and patient rights
• complete psychosocial assessments and discharge planning evaluations
• document findings and communicate needs across the care team
• coordinate patient, family, and caregiver education and family conferences.
