What you'll do

• Conduct initial and ongoing in-home assessments
• develop and update individualized Plans of Care
• supervise Personal Care Aides and Respite Care staff
• support Consumer-Directed clients and Employers of Record with education and service coordination
• complete Medicaid, DMAS, waiver, and agency documentation
• support quality improvement, chart reviews, incident follow-up, and compliance monitoring.
