MDS Coordinator

Job overview

The MDS Coordinator role is onsite in Silver Spring, MD, paying $95,000–$105,000 per year. Woodside Rehab & Nursing, a Vierra Communities property, provides health and rehabilitation services to residents and patients. The coordinator manages the facility’s Resident Assessment Instrument process and ensures assessments and supporting documentation are accurate and timely. The role works with the interdisciplinary team, health plans, physicians, and facility staff to maintain care plans, support coverage decisions, and meet regulatory and reporting requirements.

What you'll do

• Coordinate the facility’s RAI process
• complete MDS and supporting assessments and documentation
• implement and evaluate resident care plans
• schedule and lead interdisciplinary care-plan meetings
• audit medical records
• educate staff as needed
• coordinate with health plans and physicians on approvals and eligibility
• schedule assessment reference dates
• transmit assessments and resolve errors
• support admissions, discharges, care, training, and quality activities.

Why this role

Requires an active, unencumbered RN or LPN license and three years of RN/LPN experience, including supervisory experience; RAC training must be completed within six months of hire.
