The Registered Nurse (RN), Weekend Shift role is an onsite position in Green Bay, WI, covering all Unity territories. The schedule is 36 hours per week on 12-hour night shifts Friday through Sunday, 7 p.m.–7 a.m.; compensation is not specified. The nurse supports Unity Hospice’s community, Meng Residence, and Triage services. The role provides skilled nursing assessment and interventions, telephone triage, and patient and family support across care settings. It contributes to coordinated hospice care by updating care plans, responding to urgent needs, communicating with the interdisciplinary team, and supporting patient visits and transitions as staffing needs change.
What you'll do
- Rotate duties among the community, Meng Residence, and Triage based on staffing needs
- complete skilled nursing assessments and interventions
- provide telephone advice, care instructions, and direction to appropriate care
- update care plans and review them with patients and families
- respond to urgent needs and initiate crisis care as needed
- report significant findings to the primary team, attending physician, and caregiver
- educate patients and families
- administer and titrate ordered medication
- manage patient and residence contingency medications
- document orders and communications
- delegate nursing duties to LPNs and supervise CNA care
- complete admissions as needed
- coordinate volunteer deliveries.

Why this role
