The Case/Disease Management Nurse Navigator role is onsite in Wheeling, WV, with a stated schedule of 8:00 a.m.–5:00 p.m. and 40 hours. Compensation is not specified. The role supports The Health Plan’s care coordination for health plan members. The nurse navigator assesses moderate- to high-risk members and coordinates quality-based, cost-effective care tailored to member needs, benefits, and community resources. The role develops and monitors care plans, supports member decision-making, and leads multidisciplinary coordination across the care continuum.

What you'll do

• Assess moderate- to high-risk and complex cases
• coordinate case and disease management
• conduct medical, behavioral, functional, and social determinant assessments
• develop, monitor, revise, and close care plans
• coordinate providers and multidisciplinary teams
• educate and advocate for members
• arrange community services
• document interactions and case activity
• and promote quality, cost-effective interventions.
