What you'll do

• Assess new patients’ psychosocial needs
• identify and counsel on psychosocial issues
• educate patients and families
• coordinate transient arrangements
• advocate for patients with agencies
• identify and refer to social agencies and funding resources
• participate in patient assessment and care planning
• support quality improvement activities and patient/family support groups
• educate staff on psychosocial care.
