What you'll do

• Assess veteran residents’ and households’ needs and develop support plans focused on obtaining or maintaining permanent housing
• provide practical, personal, social, and general care using a trauma-informed approach
• monitor and document plans
• coordinate with agencies and accompany residents to appointments as needed
• maintain regular contact, including crisis intervention, referrals, and life-skills development
• assess housing habitability
• update HMIS data
• provide staff support while on call and help cover open shifts or call-offs when on call
• conduct local outreach
• maintain records and attend requested meetings and training.
