What you'll do

• Review admission, discharge, and transition reports and patient records
• contact patients within 24–72 hours after emergency department visits or hospital discharge
• assess follow-up needs and coordinate services using the EHR
• conduct telehealth visits when appropriate
• coordinate appointments and address barriers with primary care teams
• perform utilization reviews
• identify preventive-care gaps and conduct outreach
• participate in quality improvement initiatives.
