What you'll do

• Verify the plan of care with the RN before each visit
• collaborate with physicians and the RN and report changes in patient condition
• teach patients and caregivers self-care techniques
• check vital signs
• review medications and perform medication reconciliation at each visit
• perform physician-ordered procedures
• prepare treatment equipment and materials using aseptic technique
• and complete documentation accurately and promptly.
