The Registered Dietitian role is in person in Bremerton, WA. Compensation is $40–$45 per hour. Port Washington Post Acute provides skilled nursing and post-acute care in a supportive, team-oriented environment. The dietitian assesses and manages residents’ nutrition care to support nutritional health, quality of life, and clinical outcomes. Working with the interdisciplinary care team, the role develops individualized plans, monitors progress, and supports regulatory compliance and facility quality initiatives.

What you'll do

• Conduct nutrition assessments
• develop, implement, and monitor individualized nutrition care plans
• evaluate nutritional risk, weight changes, wounds, swallowing concerns, and therapeutic diet needs
• document resident status
• participate in care plan meetings
• provide nutrition education
• review menus
• monitor food service operations
• support quality improvement and regulatory compliance.
