What you'll do

• Review development and execution of personalized care plans
• facilitate interdisciplinary team meetings and service delivery requests
• serve as manager on call during non-operational hours and complete follow-ups
• run clinical delivery reports
• balance staffing and coverage
• coordinate care with interdisciplinary teams
• review UAS assessments
• investigate incidents and ensure documentation
• review charts and records
• coordinate transitions and long-term care services
• monitor care management activities and outcomes
• support regulatory compliance, staff development, performance reviews, and employee grievance resolution.
