Job overview

The Bilingual Community Health Worker (CHW) role is field-based, serving participants’ homes and community locations throughout the assigned Central Florida service area; it is not remote or office-based. Starting pay is $18.00 per hour, with mileage reimbursement of $0.445 per mile. Renacer Foundation works to improve health, independence, and quality of life for individuals and families throughout Central Florida. The CHW manages an assigned caseload and helps older adults navigate health and social-service needs, including barriers that can affect well-being after hospital discharge. Through home visits, assessments, education, non-clinical support, referrals, and coordination with community and healthcare partners, the role connects participants with appropriate services while maintaining accurate documentation.

What you'll do

• Conduct home visits and participant assessments
• identify social needs and barriers
• coordinate approved services
• provide health education and non-clinical emotional support
• assist with medical appointments and transportation coordination
• coordinate meal and food support
• provide basic home-cleaning assistance and medication routine support within CHW scope
• help participants navigate healthcare and benefits
• make referrals and coordinate with providers, social workers, caregivers, and community partners
• complete same-day case notes and required documentation
• maintain confidentiality and follow HIPAA and organizational policies.

Why this role

Fluent English and Spanish required; Monday–Friday, 9:00 a.m.–5:30 p.m., with 40 hours per week guaranteed; personal laptop, reliable transportation, driver’s license, and automobile insurance required; CHW certification is not required for qualified entry-level candidates.
