What you'll do

• Communicate status and participate in team/caseload activities
• provide personal care
• check home and personal supplies
• research community resources and assist with transportation access
• administer delegated medications and accompany people to appointments
• support household purchases and maintenance
• collaborate with coordinators and staff
• complete required documentation and attend meetings and training.
