The Home Health Registered Nurse, Field role is onsite and on the road in Denver, CO, serving the Denver Metro area and surrounding communities. Compensation is $50–$100 per hour. Premier Home Health Care focuses on patient outcomes and personalized care in patients’ homes while supporting clinician education, training, and autonomy. The nurse evaluates patients, develops and adjusts care plans with providers, coordinates with the home health team, and documents care to support patient progress and rehabilitation.

• Evaluate patients initially and throughout care

• instruct patients, families, and team members
• observe and report patient responses and changes
• coordinate services
• communicate care plans and progress
• maintain timely documentation
• manage an agency caseload
• and participate in education and quality programs as requested.
