What you'll do

• Conduct health histories and physical exams
• provide patient teaching
• order, interpret, and follow up on lab tests
• perform selected procedures
• document care
• initiate emergency measures and identify conditions requiring referral. Manage selected medication and diagnostic orders within approved protocols, reconcile medications for skilled nursing facility admissions, visit participants in homes and hospitals as directed, request consultations, collaborate with healthcare professionals and families, participate in care planning and interdisciplinary meetings, and participate in the Primary Care on-call rotation.

Why this role
