What you'll do
- Obtains data and assesses necessary information from patients.
- Initiates communication with the attending physician and interdisciplinary team.
- Provides regular visits to re-assess patient needs and update the plan of care.
- Performs therapeutic, preventative, and rehabilitative nursing procedures.
- Performs clinical pronouncement of death.
- Maintains accurate patient records.
- Supervises and documents delivery of care by the Home Health Aide.
