What you'll do

• Complete psychosocial assessments
• develop and coordinate individualized discharge plans
• engage families and community supports as clinically appropriate
• provide family or support sessions
• obtain authorized collateral information
• participate in interdisciplinary rounds and treatment meetings
• coordinate with clinical and community providers
• maintain timely clinical documentation
• participate in required training and maintain professional licensure.
