The In-Home Dialysis Care Partner position is onsite in San Tan Valley, AZ, supporting one adult client at home. Pay is $25 per hour, and the schedule is four days a week. Herewith Caregivers provides clinical team guidance for the patient’s transition to home dialysis. The care partner helps prepare the treatment environment, provides companionship and light daily-living support, and monitors safety. The role contributes to a consistent, safe home routine as the client transitions from clinic-based dialysis.

What you'll do

• Assist with dialysis treatment setup using the Tableau dialysis machine
• keep the treatment environment clean, organized, and safe
• provide companionship and reassurance
• help the client stay on treatment schedule
• provide light ADL support as needed
• monitor safety and report concerns to the coordinating nurse
• maintain documentation and shift notes.
