What you'll do

• Conduct in-person initial assessments, reassessments, quality-assurance visits, and supervisory visits of field staff
• assist with developing, implementing, and revising patient care plans
• ensure caregivers adhere to care plans and physicians’ orders
• assist DPS and Coordination in recommending caregivers based on patients’ clinical and psychosocial needs
• communicate with team members about care quality.

Why this role
