The Clinical Team Lead provides clinical guidance, direct support, and coordination for adults with behavioral health needs. The role helps maintain quality care through assessment, treatment planning, documentation review, team development, crisis planning, and coordination with community partners.

What you'll do

• Provide clinical leadership and interventions for Community Support Team members and clients
• complete and coordinate assessments, updates, treatment plans, and recommendations
• review documentation for quality, accuracy, timeliness, and compliance
• facilitate team meetings and case reviews
• determine appropriate service intensity
• monitor progress and continuity of care
• support complex clinical decisions
• coordinate with hospitals, care managers, referral partners, and providers
• assist with crisis prevention, stabilization, transitions, and discharge planning
• identify barriers and develop solutions.
