What you'll do

• Provide general medical care and care coordination for member populations
• conduct histories, physical exams, and preventive assessments
• address acute and chronic primary care concerns
• document diagnoses and implement care plans
• order or perform appropriate diagnostic testing
• coordinate post-discharge care
• conduct in-person and alternative-modality visits
• participate in pop-up clinics
• provide alternating on-call coverage for urgent lab results and pharmacy inquiries
• document care in the EMR
• travel locally, including regular driving up to 120 miles per day.
