Behavioral Health Counselor

Job overview

The Behavioral Health Counselor role is onsite in Chicago, IL, and full-time. Compensation is $23–$25 per hour. The counselor supports the behavioral health unit at Alden Lakeland Post-Acute Rehabilitation & Health Care. The role provides psychosocial rehabilitation and clinically relevant interventions for residents, contributing to treatment planning, crisis response, and continuity of care. Counseling, case management, and discharge planning help address residents’ behavioral health needs and support the unit’s overall functioning.

What you'll do

• Provide or assist with psychosocial rehabilitation, individual and group counseling, assessment, treatment planning, MDS, discharge planning, case management, linkage, and referral
• respond to crises according to facility policy
• document services
• assist with comprehensive assessments and treatment plans
• participate in team meetings and supervision
• report suspected abuse or neglect
• respect resident rights
• assist with behavioral health unit operations. Requires a bachelor’s degree in psychology, social work, or a related field and at least one year working with a mentally ill or similar population.
