The purpose of this role is to provide specialized nutrition counseling, diabetes education, and meal planning to patients within the Food Rx program. The Dietitian works as part of an interdisciplinary team to coordinate care, manage patient enrollment, and implement evidence-based strategies to improve health outcomes for individuals with complex medical and social needs.

What you'll do

• Provide nutrition counseling, diabetes education, weight management strategies, and meal planning.
• Coordinate with Community Health Advocates and providers in the Collaborative Care Program.
• Identify eligible patients for the Food Rx program and manage enrollment and follow-up.
• Report to the Regional Clinical Manager for Food Rx.
