Perform psychosocial assessments for patients and/or families. Formulate, develop, and implement individualized treatment plans. Collaborate Lindner Center of Hope (LCOH) social workers, psychologists, psychiatrists and other treatment team members. Maintain knowledge of external resources for patient needs.
