Care Coordinator - CPST (Residential Facility)

The Care Coordinator - CPST (Residential Facility) role is an onsite, full-time position in Columbus, OH, with pay of $19.00–$24.00 per hour and a Sunday–Thursday schedule from 11:00 AM to 7:00 PM. The role supports youth in a residential treatment program. The coordinator links youth, families, placing agencies, OhioRISE, community providers, and the treatment team to coordinate services, provide medically necessary CPST, support treatment and discharge planning, and contribute to successful treatment outcomes.

What you'll do

• Maintain an assigned youth caseload and provide CPST when identified in the treatment plan and medically necessary
• conduct sessions with youth and collateral contacts
• coordinate referrals, appointments, and aftercare
• participate in treatment, family, and case meetings
• communicate with families and providers
• complete progress notes and reports
• report urgent safety concerns and use trauma-informed care and de-escalation techniques.
