The Case Manager / Independent Living Specialist / Parent Trainer role is onsite in Los Angeles, CA. Pay is $18.42 per hour. The position helps individuals build daily living skills, access benefits and community resources, make decisions, and receive coordinated support. It also documents services and contributes to service-plan reviews and meetings.

What you'll do

• Support development and implementation of individual service plans
• provide in-home daily living skills training and community resource and transportation training
• assist with benefits access and reporting
• monitor health and safety needs
• advocate, support goal-setting and decision-making, and respond to crises
• document services, prepare reports, recommend support, and participate in IPP meetings.
