What you'll do

• Participate in placement matching and intake
• develop treatment plans and coordinate assessments and referrals
• support foster children and parents
• monitor required services and documentation
• coordinate with agencies, schools, families, and care teams
• complete home visits and transport clients as needed
• participate in a weekly rotating on-call schedule
• document incidents and Medicaid services.
