Hospice Registered Nurse (RN), Case Manager

What you'll do

• Assess patients and families to determine home care needs
• reevaluate nursing needs
• initiate and revise plans of care
• develop nursing diagnoses and care goals
• administer prescribed medications and treatments
• counsel and educate patients and families
• identify and implement discharge planning needs
• coordinate assigned caseload when acting as case manager
• prepare clinical notes and communicate changes to physicians
• coordinate with community health partners
• participate in on-call duties
• arrange necessary equipment and services
• instruct, supervise, and evaluate home health aide care every two weeks.
