Social Work Care Manager & Patient Advocate (LSW/LCSW)

The Social Work Care Manager & Patient Advocate (LSW/LCSW) role is hybrid and field-based, with a Paramus, NJ listing location and regular travel for client visits. Base pay is $75,000–$85,000 per year, plus a monthly performance bonus based on billable hours. Unicity Care Management supports individuals and families navigating complex healthcare decisions. The care manager serves as a primary contact for clients and families, assessing needs, coordinating providers and community resources, supporting care transitions, and advocating for clients at appointments and care decisions. The role includes responding to urgent needs outside standard business hours.

What you'll do

• Conduct in-home functional, cognitive, psychosocial, and environmental assessments
• develop and update individualized care plans
• coordinate physicians, home care, specialists, and community resources
• support transitions of care
• provide emotional support and crisis intervention
• advocate at appointments and care conferences
• serve as the primary contact for assigned clients, including urgent needs outside standard business hours
• and accompany clients to medical and other care-related visits as needed.
