The role supports coordinated, compliant care and housing stability through assessments, supervision, quality assurance, crisis support, and collaboration with community providers.

What you'll do

• Conduct initial biopsychosocial assessments and review annual reassessments
• supervise, train, and evaluate case managers and an MSW intern
• oversee clinical documentation, training, quality assurance, and contract compliance
• coordinate service delivery and staffing
• support crisis management on site and on call
• facilitate team meetings and specialized groups
• coordinate intake and diagnostic screening
• monitor evidence-based services and regulatory compliance.
