Manager, Care Management

Job overview

The Manager, Care Management is an onsite, full-time position in Johnston, RI, supporting telephonic and in-home care management services. Pay is $100,000–$150,000 per year. The role supports Astrana Health’s Health Services Outpatient Care Management team and its work with high-risk patients. The manager leads a multidisciplinary clinical team and oversees care management operations, staffing, training, compliance, and program effectiveness. The role exists to help prevent unnecessary utilization, improve patient outcomes, and advance value-based care for complex patient populations.

What you'll do

• Manage clinical services, policies, procedures, and daily staff activities
• ensure adequate staffing and workload distribution
• train staff on engagement, NCQA, and HEDIS processes
• maintain compliance with HIPAA and applicable regulations
• monitor outreach and intervention effectiveness
• ensure appropriate care planning and interventions for at-risk members
• coordinate information exchange between behavioral and medical providers
• monitor quality data and service gaps
• and improve care management programs.

Why this role

Requires a nursing or related bachelor’s degree or graduation from an accredited nursing school, Rhode Island licensure, and at least three years of care management or utilization management experience. Occasional local travel may be required.
