What you'll do
- Support primary care under a planned care model.
- Triage patients in person and over the phone.
- Collaborate with PCPs to identify preventive care needs.
- Provide health education during visits and via telephone.
- Monitor patient self-management goals.
- Coordinate care with community-based providers.
- Assist providers with exams and procedures.
- Perform prenatal NSTs.
- Administer medications and immunizations.
- Document patient care in the EHR.
