Transportation Driver — Skilled Nursing

What you'll do

• Coordinate and transport residents to medical appointments, outpatient programs, community facilities, and from hospitals or other facilities
• follow a daily schedule based on resident needs
• observe residents and report condition changes
• assist with transfers and mobility as directed
• maintain the vehicle
• coordinate with outside providers
• notify nursing leadership of schedule changes
• provide hydration and protect belongings and confidentiality.
