What you'll do

• Ensure safe, quality hospice nursing services in patients’ homes or assisted-living settings
• develop, implement, and evaluate plans of care and revise them as needed
• coordinate care with other health professionals
• identify interdisciplinary needs and recommend additional home-health services
• supervise care provided by Licensed Practical Nurses and Home Health Aides
• educate and counsel clients and families
• and complete timely, accurate electronic documentation.
