CCBHC Care Coordinator

Job overview

The CCBHC Care Coordinator role is onsite in Portland, ME, and pays $23–$26 per hour. The position supports Spurwink’s interdisciplinary team serving adults with behavioral health challenges in the greater Portland area. The coordinator assesses client needs, develops integrated care plans with the team, and monitors implementation and progress. This work helps coordinate behavioral and physical health goals through collaboration with clinical, peer, nursing, psychiatric, and medical colleagues.

What you'll do

• Conduct intake and ongoing assessments
• facilitate team meetings and develop client care plans, including crisis plans when appropriate
• oversee care-plan implementation and document progress
• collaborate with the Clinical Team Leader, Peer Partner, Nurse Care Manager, and psychiatric and medical consultants.
