What you'll do

• Plan and evaluate service plans and monitor objectives
• document services and write weekly progress notes
• conduct daily site visits and monitor site maintenance
• oversee medication maintenance and support self-medication skills as needed
• connect residents with community resources, vocational assessment, job training, and education
• provide advocacy, mediation, counseling, behavioral intervention, and crisis management
• coordinate with families and providers as needed
• provide transportation as needed.
