UR Medicine Home Care provides care to patients throughout its community. The nurse assesses patients, develops and initiates appropriate care plans, and documents OASIS information reflecting clinical, functional, and psychosocial needs. The role coordinates with patients, families, physicians, and the interdisciplinary team to support quality outcomes, patient education, and continuity of care; on-call, evening, or weekend visits may be needed.

• Conduct comprehensive patient assessments, including OASIS documentation, and develop individualized care plans

• maintain timely clinical documentation
• educate patients and caregivers and connect them with community and healthcare resources
• communicate changes in patient status
• manage time, resources, and productivity
• participate in on-call, evening, or weekend visits as needed
• maintain professional competency through continuing education.
