What you'll do

• Provide outpatient primary care to seniors with complex chronic diseases
• manage a patient panel of up to 450, with 18–22 patients per day
• work with a dedicated medical assistant, scheduler, and referral specialist
• use scheduled administrative time to support care
• collaborate with in-house consulting specialists and holistic health services.
