What you'll do

• Direct, supervise, and ensure quality of care provided by Coordinators of Care and clinical staff
• oversee periodic reassessments and care-plan revisions
• ensure timely recertification packages
• evaluate staff skills and services through record review, field visits, and case conferencing
• communicate with staff and patients
• advocate for patients with third-party payers
• identify staffing and service needs
• participate in hiring, training, discipline, orientation, education, and performance improvement
• participate in on-call coverage rotation.
