What you'll do

• Conduct in-home assessments
• develop and maintain care plans in AxisCare
• supervise care professionals, including first-shift presence for new client placements
• conduct 30-day supervisory visits
• monitor care quality and respond to concerns
• maintain AHCA compliance documentation
• coordinate scheduling, client-caregiver matching, and shift continuity.
