Transition Coordinator — Student Transition Services

Job overview

The Transition Coordinator role is onsite in Bourbonnais, IL, with general staff hours of 7:30 a.m.–3 p.m. Monday–Friday; occasional responsibilities may extend beyond work hours. Pay is $40,000–$75,000 per year. The role combines student coaching, transition assessments, compliance documentation, and partnerships with families, employers, and community agencies.

What you'll do

• Supervise and coach students
• support self-advocacy and person-centered IEP transition meetings
• assess student abilities, interests, independent living, transit, and work behaviors
• draft measurable transition goals in line with IDEA mandates
• track progress
• develop community-site relationships, work-study, internships, and volunteer opportunities
• connect families with agencies
• support students at work-based placements.
