What you'll do

• Coordinate and manage the MDS assessment process, identifying and addressing issues or concerns
• collaborate with the healthcare team to complete assessments accurately and efficiently
• develop individualized resident care plans
• educate and support residents and families to promote independence and quality of life
• identify potential quality-of-care issues
• maintain detailed records of assessments, care plans, and resident progress. Requires a Nevada RN license, MDS experience, and long-term RN experience
• an MDS license in Nevada is preferred.
