Nurse Care Manager

Job overview

The Nurse Care Manager role is onsite Monday through Friday at Privia Health’s CVFP New London Office in Forest, VA. The annual base salary range is $64,800–$72,000, with an annual bonus targeted at 10%. The role supports Privia’s highest-risk patients in collaboration with population health teams, facility partners, primary care providers, and practice operations. The nurse coordinates care, conducts holistic assessments and Medicare Annual Wellness Visits, and helps patients follow care plans and access appropriate resources. The work aims to improve clinical outcomes, close quality gaps, and reduce high-cost utilization.

What you'll do

• Manage a caseload and follow up on care plans, appointments, and outcomes
• engage patients by phone and in person
• liaise between patients and primary care providers
• conduct holistic assessments and develop care plans
• perform Medicare Annual Wellness Visits, including vital signs, vaccines, and review of histories, medications, and preventive needs
• educate patients on chronic disease management
• use Athena and Privia applications to identify utilization and quality opportunities
• comply with HIPAA.
