What you'll do

• Triage referrals and coordinate clinical reviews
• verify payer sources and obtain authorizations
• collaborate with nursing leadership and the interdisciplinary team on clinical appropriateness and capacity
• serve as liaison to hospitals, case managers, discharge planners, and referral sources
• communicate bed availability and resident status
• support discharge planning and transitions
• maintain referral, admission, census, and occupancy records
• develop outreach strategies.
